Sub: Submission of Life Certificate/digital Jeevan Pramaan by the Pensioners and Members of the EPS — 1995- regarding.

Sir/Madam,

Attention is drawn to this office circular No. Pension --I/17(10)16-17Jeevan Pramaan/22750, 613, 31402 dated 28.11.2016, 16.01.2017 & 15.02.2017 respectively on the above cited subject. 

2. Now it is decided that it will be mandatory for all pensioners of the EPS' 1995 who are desirous of continuing the benefits of the said scheme to furnish their Jeevan Pramaan/Life Certificate by 30.04.2017. 

3. In case the pensioner is not able to submit the Jeevan Pramaan Patra/Life Certificate in digital form, the same be obtained in paper/physical form along with his/her clarification about the reasons for not submitting the same in digital form. On submission, the same may be accepted on valid grounds. 

4. The pension in respect of those pensioners who do not submit either the digital Jeevan Pramaan patra or the Jeevan Pramaan patra in physical form as stated above should be stopped from the month of May 2017.
